- 1. Contact any one of our offices via telephone or fax.

- 2. Our pre-cert department will contact patients for insurance information and coordinate
with insurance companies and physicians to obtain authorizations, if required.
Patients will
then be scheduled at the time and location most convenient for them.
Our goal is to initiate therapy within 24 hours.
- 3. Patients will be evaluated for physical or occupational therapy and treated per physician
referral.
The Plan of Care and follow-up reports will be communicated to the referring physician.
